For Aviators, Log into FAA medxpress to fill out your portion of the application. Print it and bring it with you.

Please fill out your medical history from infancy to present, including if you have had any recent surgeries 

and hospitalizations.  Bring your imaging reports and treatment records with you, as these will be scanned to the FAA.  Our goal is to make your experience easier.  We are here to advocate for you and get you flying.

Currently we are only performing 2nd class and 3rd class FAA physicals.
